Skilled Nusing Care

We are committed to excellence in our field. We provide compassionate care for all of our residents to optimize a life of dignity and physical well being.

The personal care plan of each resident is designed and carried out by a  team of medical professionals including nursing, social work, activities, dietary, and rehabilitation. We provide a full time Pastoral Care program. This is the only skilled nursing facility in the region to offer Music Therapy.  The Lutheran Home is fully certified and licensed by the New York State Department of Health and is recognized by Medicare, Medicaid, and many third party insurance carriers.

In addition to the services required by the certifying agencies, the Lutheran Home offers enhanced care in several areas.

The Home offers a state of the art Rehabilitation Program, which includes physical therapy, occupational therapy, and speech and swallowing therapy. Our advanced clinical services include; intravenous (IV) medication and feedings, respiratory care, post-stroke and post-surgical rehabilitation, dementia care in our new Bridges Program, Hospice Care, Pain Management, and advanced wound management programs. We serve those in need of short stay and rehabilitation services. For residents choosing palliative care, we participate with the Chautauqua County Hospice. A full time Lutheran chaplain is available to all residents, although we promote and facilitate the spiritual care for all residents through their preferred tradition.

    Pastoral Care at Lutheran

    The Pastoral Care Program is dedicated to serving the religious and spiritual needs of all our residents. These needs are met through ecumenical worship services, Bible study, prayer, counseling and visitation. As a ministry of the Evangelical Lutheran Church in America, Upstate New York Synod, Lutheran was founded as a place where the needs of the whole person are ministered to with dignity and respect for each person’s religious and spiritual beliefs.

    Worship in Christ Chapel

    Christ Chapel offers a beautiful and central worship space for residents of Lutheran. Our Chaplain leads weekly Sunday services that are open to all. Worshippers include residents and family members of the Lutheran Home and Rehabilitation Center, Hultquist Place, and Lutheran Senior Housing, as well as, visitors from the community.

    Additional services are scheduled during Lent and special holidays. Holy Communion is scheduled the first Sunday of every month, and weekly during Lent. Mass is conducted monthly by a Roman Catholic Priest for our many Roman Catholic residents. Eucharistic ministers also bring Communion to Catholic residents on a regular basis.

    10:30 am Roman Catholic Mass (2nd Friday of each month)

    Worship services are broadcast live in the facility on Chanel 10 for residents who prefer to stay in their room for worship.

  • Bible Study

    Bible Study

    Bible studies are held at numerous times and locations within the Lutheran Home during the week to meet the schedules of our residents. Specific days and times can be found on the Activity Department monthly calendars or by speaking to activity or pastoral staff.

  • End-of-Life Support

    End-of-Life Support

    Lutheran staff show special attention to support the end-of-life needs of residents and their families. From medical care, pain management, personal, emotional and spiritual support, the staff works as a coordinated team to make the dying process as comfortable and dignified as possible. We also often partner with Hospice of Chautauqua County to add an additional level of compassionate care and support.
